OFFICIAL RULES AND REGULATIONS – Atlanta Hawks vs. Boston Celtics VIP Ticket Promotion

ELIGIBILITY: The Sweepstakes is open to legal residents of the United States and the state of Georgia who reside within 75 miles of the city of Atlanta and are twenty-one (21) years of age or older as of the date of entry. Employees (and members of their immediate families or household) of Woolfson Eye Institute (the "Sponsor"), its franchisees, or of any of their respective parent companies, subsidiaries, affiliates, advertising and promotional agencies, and/or sweepstakes promotional partners, and any other person or entity involved in the design, execution or judging of the Sweepstakes are not eligible, which shall include Atlanta Hawks, L.P., (“AHLP”) Atlanta Hawks Management, LLC, Arena Operations, LLC, Atlanta Spirit, LLC, the National Basketball Association and its Member Teams (“NBA”), NBA Properties, Inc., and their respective parents, subsidiaries, divisions, directors, officers, members, partners, trustees, franchisees, governors, agents, owners, shareholders, participating vendors, distributors, advertising and promotion agencies and affiliated entities employees and each of their respective families (spouses, siblings, children, household members and the spouses and children of the foregoing).


HOW TO ENTER: The Sweepstakes begins 6:00 a.m. October 28, 2009 and ends 5:00 p.m. December 31, 2009 (otherwise known as the "Entry Period"). There are two ways to enter. From October 28, 2009 until December 31, 2009, everyone who competes a WOOLFSON LASIK procedure will be automatically entered. Or, hand print your full name, age, home address, zip code, and daytime phone number (with area code) on a 3" x 5" postcard (no envelopes) and mail with proper postage via U.S. mail to: Woolfson Eye Institute c/o T.Vanderbilt, Sweepstakes Entry, PO Box 89132, Atlanta, GA 30312. Online entry ends at 5:00 p.m. EST on December 31, 2009. Mail entries must be postmarked by December 31, 2009.

 Sponsor is not responsible for misdirected, late, incomplete, illegible, or lost entries, or any unforeseen circumstances, which may limit an individual's ability to participate in the Sweepstakes. All entries become the property of the Sponsor and will not be returned. Limit one online entry per household. Mailed entries must be sent separately and bear sufficient postage.



WINNER SELECTION: The odds of winning depend upon the total number of eligible entries received during the Entry Period. Prize Winners will be selected during a random drawing conducted on or about January 11, 2010 for 2 VIP Tickets to the Atlanta Hawks vs. Boston Celtics Game on January 29, 2010 Sweepstakes. T.Vanderbilt, a marketing organization, whose decisions will be final with respect to all aspects of the Sweepstakes, will hold on or about January 11, 2010, the Grand Prize drawing for 2 VIP Tickets to the Atlanta Hawks vs. Boston Celtics Game on January 29, 2010. Prize winner will be notified via telephone and U.S. mail within three (3) days after the drawing date and must complete an Affidavit of Eligibility and Liability Release, which includes the winner's tax identification number for tax reporting purposes. Non-compliance may result in disqualification and selection of an alternate winner. Return of any prize notification as undeliverable may result in disqualification and selection of an alternate winner solely at the Sponsor's discretion. Sponsor will use reasonable efforts to select an alternate winner but in the event one cannot be reasonably determined, such prize will not be awarded.

 


PRIZES: One (1) Grand Prize of 2 VIP Tickets to the Atlanta Hawks vs. Boston Celtics Game on January 29, 2010 will be awarded. The Grand Prize consists of 2 Tickets to the Atlanta Hawks vs. Boston Celtics game on January 29, 2010. Approximate Retail Value of the Grand Prize is $140.00. 
Sponsor reserves the right to substitute any prize(s) of like value in the event the described prize(s) or portion thereof are unavailable for any reason whatsoever.

GENERAL CONDITIONS: Contest offered to eligible participants, but void where prohibited or restricted by law. By submitting an entry, you agree to release Woolfson Eye Institute, its franchisees, all participating retailers, advertising agencies and their respective officers, directors, employees, agents, parent companies, affiliates and subsidiaries from liability of any kind or nature for any loss, claims, damages, or injuries of any kind associated with participation in sweepstakes or acceptance, possession, participation in or use of any prizes which shall include Atlanta Hawks, L.P., (“AHLP”) Atlanta Hawks Management, LLC, Arena Operations, LLC, Atlanta Spirit, LLC, the National Basketball Association and its Member Teams (“NBA”), NBA Properties, Inc., and their respective parents, subsidiaries, divisions, directors, officers, members, partners, trustees, franchisees, governors, agents, owners, shareholders, participating vendors, distributors, advertising and promotion agencies and affiliated entities employees and each of their respective families. You accept and agree to be bound by these rules and the decisions of the Sponsor relating to participation in Sweepstakes, selection of winners and awarding of prizes, whose decisions are final. Acceptance of prize constitutes permission to use winner's name, biographical information, and/or likeness for purposes of advertising and promotion without further compensation, except in Tennessee and where prohibited by law. Any Sweepstakes materials (including without limitation, offer, rules and announcement of winners) containing production, printing or typographical errors or obtained outside authorized legitimate channels may be declared void at the discretion of Sponsor; and liability is limited to replacement of such materials. Recipient of any such materials agrees to release Aaron Rents, Inc., its franchisees, all participating retailers, contest promoter, advertising agencies and their respective officers, directors, employees, agents, parent companies, affiliates and subsidiaries from any and all losses, claims or damages that may result in connection with any such materials. The Sponsor expressly disclaims any responsibility, and entrants agree to hold Sponsor, harmless for liability for injury or loss to any person or property relating to participation in this Sweepstakes or the delivery and/or subsequent misuse of any of the prizes awarded.
